Supporting the development of a new Symphony Hub in Yeovil District Hospital
Funding was allocated to the South Somerset based Symphony hub to provide education and training. The training was for 2 new roles, key workers and health coaches. The key worker training is predominantly a Level 3 Qualifications and Credit Framework (QCF) in health and social care plus other internal training sessions that covers health coaching skills, care coordination & processes, team development and support skills.
An internal Training, Assessment and Quality Assurance (TAQA) assessor was also appointed to support the QCF training. The key workers are part of the complex care hub and liaise with GPs, consultants and the nursing care co-ordinators.
