Leading systems transformation

The Sustainability and Transformation Plans provide us with a great opportunity to think more about population health and preventative healthcare, to look at our health and social care system as a whole rather than a group of separate health care providers. The leadership skills required to do this place a much greater emphasis on the ability to work and lead across a healthcare system, building relationships with citizens and across communities, organisations and sectors, using innovation and improvement models to make the service better for patients and sharing best practice.
This award is for individuals/teams who have demonstrated these essential qualities of leading change across systems, demonstrating effective systems leadership as described below.
Please answer the following questions by giving suitable examples of how the nominee(s) demonstrated leadership qualities in their work, words or actions. We have made some suggestions of examples you might give; do feel free to use any others which you believe answer the question.

Q1 How do their relationships and connectivity contribute to Systems Leadership within their area? Examples of this are:
- They develop strong and honest relationships that have brought people together to create new cultures and agree values and behaviours
- They understand and value others
- They bring clarity to a situation when things are unclear
Q2 How does their individual effectiveness contribute to Systems Leadership within their area? This may be shown through:
- Stories of how they manage organisational and professional boundaries across the system while maintaining an inclusive approach
- Demonstrations of patience, persistence and resilience
- Stories of how they operate comfortably and skilfully in complex/ uncertain environments
Q3 How does their understanding of Innovation and Improvement contribute to Systems Leadership within their area? Here, we are looking for examples such as:
- A demonstration of their understanding ‘improvement science’ with a focus on continual improvement
- How they role model innovative ways of “thinking and being”
- How they encourage others to create innovative solutions to complex problems
Q4 Do they demonstrate how Learning and Capacity Building contributes to Systems Leadership within their area? For example:
- They are able to share and transfer ‘good practice’
- They have an inclusive approach to talent management
- They show a good understanding of developing others
- They engage with patients, citizens and communities
Q5 Do they demonstrate inclusive leadership? Examples of inclusive leadership may be:
- They are an ambassador, advocate or ally for the under-represented or unheard. They may be part of, or run, a network or representative committee and unlock potential through their development of inclusive services and in the process help to shift their organisation’s culture
- They inspire by helping others ‘be themselves’, demonstrating an interest in and treating all with compassion, care and professionalism regardless of who they are, background, or job role. This may be seen through staff and/or patient feedback/surveys or be from your own experiences of working with them
- They ‘call out’ bias, exclusion and discrimination when and wherever they see it, enabling teams, services, organisations and systems to continually develop inclusive practices, modelling conscious, intentional, inclusion
