Foundation training pharmacist programmes forms

Below are forms that can be completed to support users of the foundation training pharmacist programme (FTPP) as the foundation trainee pharmacist comes to the end of their first placement. The forms are designed to support foundation trainee pharmacists, designated supervisors and designated prescribing practitioners:

Handover review form between DPPs (Word document only, 80KB): The purpose of this form is to allow for one DPP to give another DPP an update on the foundation trainee pharmacists progress, in the event that there is more than one DPP or there is a change in DPP.

DPP to DS meeting form (Word document only, 77KB): The purpose of this form is to provide a structured way for the DPP to provide an update to the DS on the foundation trainee pharmacists progress. This can be used as an audit tool.

Prescribing development review (Word document only, 33KB): This form is to be completed by the foundation trainee pharmacist with the DPP to reflect on the FTP’s progress with the assessment activities in relation to prescribing.

E-portfolio tips for users

• Change in Designated Supervisor: If you are a trainee and have a change in Designated Supervisor, you must send a completed copy of the GPhC Change of foundation training details form to prereg@pharmacyregulation.org and england.traineepharmacist@nhs.net. Within the email please confirm that you are a Bradford Sandwich course user.
• Change in Designated Prescribing Practitioner:  If you are a trainee and have a change in Designated Prescribing Practitioner, email england.traineepharmacist@nhs.net
• Locked or suspended accounts or other functionality issues: Please contact the RPS at eportfolio@rpharms.com  or on 0207 572 2737 (9am to 5pm, Mon-Fri). Within the email please confirm that you are a Bradford Sandwich course user.

Assessment forms and video guides

We have developed a series of training videos and templates to help trainees and their designated supervisors collect evidence. Refer to E-portfolio and downloadable resources | NHS England | Workforce, training and education to explore these further.

Due to phased developments, the screen shots within the videos may appear slightly different to your current E-portfolio, but this does not fundamentally change the functionality or guidance. The videos and guidance will also include the former Health Education England logo. There also won’t be reference to the prescribing related activities within the videos.

Getting started

• Learning Needs Analysis (LNA) – For assessing learning needs to review the trainees perceived level of learning/competence to date and formulate a plan for the next period of training. Available is an LNA Guide to support the use of the LNA in the E-portfolio.
• Bradford Sandwich E-portfolio User Guide suitable for trainee pharmacists, Designated Supervisors, Designated Prescribing Practitioners and Educational Programme Directors/Educational Leads.
• Bradford Sandwich E-portfolio: Collaborator Guide suitable for collaborators, for example, Practice Supervisors.
